Provides medication therapy resources to patients, staff and physicians. Assesses new and ongoing drug therapy for appropriateness, safety and ability to produce optimal outcomes for each individual patient. Leads and checks work of Pharmacy Tech.

Assists in formulary management by implementing approved therapeutic substitutions, dosage adjustments and stop dates. Recommends appropriate therapeutic alternatives, and monitors patient outcomes. Processes medication orders in a timely, efficient and accurate manner. Leads and checks work of Pharmacy Tech as directed by or in the absence of Pharmacy Manager.


Key Tasks & Responsibilities


Essential Functions

  1. Prioritizes medication orders.
  2. Carefully checks each medication order. Utilizes patient history (where appropriate), age, weight, height, laboratory data, and allergies to ensure that it is optimal and individualized for the patient.

  3. Reviews patient profiles (for drug interactions, allergies, therapeutic duplication, appropriate medication route, etc.) prior to entering new medication orders.

  4. Processes medication orders in a timely, efficient and accurate manner.

  5. Provides appropriate and accurate information regarding drug therapy to physicians, nurses, patients and employees.

  6. Reports Adverse Medication Events.

  7. Suggests the conversion to oral dosage route when appropriate.

  8. Suggests appropriate formulary/therapeutic options to non-formulary medications.

  9. Carefully checks all work performed by technicians.

  10. Leads the work of technicians as directed by or in the absence of Pharmacy Manager.

  11. Maintains accurate controlled medication records, including following proper procedures and record keeping for the dispensing, return, and destruction of controlled medications.

  12. Collaborates with nursing staff to ensure floor stock levels and product are appropriate, and that the overall pharmaceutical needs of the unit are met.

  13. Prepares special and complex medications accurately (e.g. IV admixtures and extemporaneous dosage forms).

  14. Serves as a resource to technicians, employees and patients.

    Follows all departmental policies and completes assigned duties within specified time frames.


Non-Essential Functions

  1. Performs other duties as assigned.
  2. Actively supports and upholds the mission and core values of the Hospital.
  3. Remains knowledgeable of and follows the policies of the Hospital.
  4. Maintains patient and hospital confidentiality at all times.
  5. Informs Manager of applicable day-to-day situations.


Qualifications & Requirements

One year experience in acute care facility, current licensure as a pharmacist in good standing within state of Kansas. Master's (MS, MPH, MHS, MBA) or Pharm D. degree.
